Mandated Health Examination

California law mandates a CHDP (Child Health and Disability Prevention) health examination. By law, all children entering the first grade are required to have either a certificate of a CHDP health examination or a CHDP waiver (green form) on file at the school in which they enroll. School health examinations must be completed within 18 months prior to, or 90 days after first grade entry unless the parent or guardian signs a waiver stating they do not want, or are unable to obtain, an examination for their child. The examination can be done by the family’s own doctor or by making an appointment for a health examination at their local County Health Department. Their child may qualify for a CHDP school entrance exam at no cost to them depending on their income and family size, if the County Health Department does the exam.
