CWCS is attempting to prevent dual enrollment.  Two(2) late notices for the drop (Notification of Withdrawal) slip have been sent to you regarding the following family:

If the drop slip (Notification of Withdrawal) is not received by the Placerville office within three(3) business days this family will need to be dropped from our school.  The family will need to take our “Notification of Withdrawal” form to their previous school and have the last day of attendance entered by the office staff and sign and date the section under the section “School Authorization”.

The completed form needs to be faxed either by the family or by you to 530-295-3583, Attention: Student Assignments.

If the family had previously filed an R4, they would submit the form with the parent signature as the office staff.
