Revised 04/2007

CWCS Student Application

A Student Enrollment Application must be completely filled out with all elements submitted before a student can be considered as a candidate for enrollment in CWCS. A copy of the birth certificate, immunization records, a copy of a current IEP if special ed, a copy of current transcript for all high school students, and CAHSEE scores for grades 11 and 12, are all required before the application is considered complete.

Submission of the application does not constitute enrollment in the school.

The information contained in the enrollment application is used to assign an Education Specialist to the student. CWCS enrolls new students through a Centralized Intake Process, and siblings can only be enrolled once they are on their ESs prospective list.

If enrolled, the student records office will automatically request a cume folder from the student's previous school.

If a parent falsifies information on an enrollment application, and it is later determined either via ES or cume that the the information is incorrect, the student will be dis-enrolled for falsifying the application. If they wish to re-apply, they may due so by contacting the school executive director.
