___1. The Assessment Confirmation Form has been explained and signed, and I understand that missing any required school or state test session constitutes a truancy with this school.

 

___2. I have been provided with the California Content Standards for the grade level of my student for this year.

 

___3. The optional Benchmark Exams and the four required Writing Prompt samples have been explained. A writing sample will be collected today.

 

___4. I have been provided with a copy of the School Calendar.

 

___5. I have been provided with a copy of the ESLRs and completed the ESLRS survey.

 

___6. The truancy, suspension, and expulsion policies have been explained.

 

___7. Instructional Funding available for my student this year has been discussed and spending priorities have been set. I understand that the school’s funding can not be used to purchase denominational or sectarian materials, and that all educational materials provided to my students remain the property of this charter school. I have received a Notification of Withdrawal form to be filled out by my previous school.
