CWCS K-8 Sibling
Enrollment Meeting
(to
be used only for siblings of currently enrolled students)
1. Phone Interview:
The CWCS Phone Interview Questions is a list
of questions to discuss over the telephone prior to the enrollment meeting.
As soon as the student is on your prospective list in FRED,
call the parent to discuss the interview questions. Please use the family’s
responses to these questions to counsel them as to whether or not this school
can meet their expectations prior to completing enrollment paperwork. If you
are having a conversation regarding a prospective student, and something concerns
you, you need to tell the parent your concern, and give them a chance to address
the issue. Consult your advisor with any concerns before making an appointment
to enroll the student. The parent should not be surprised that you have a
concern, but be a part of deciding if this school will work for them. Even
though the family has students currently enrolled in our charter school, some
of these questions may pertain to the new student that may not pertain to
the siblings.
2. Before
the Meeting:
For
the initial enrollment meeting, print the following forms. If printed from
FRED, see “Where to find FRED forms”
for directions. Because the student is a sibling of active students many of
the forms from the First
Meeting Information Sheet were already given to the parent at the beginning
of the year, only the following are needed for the sibling enrollment.

Assessment
Confirmation Form (please be careful to use the words: “All students
must take the Mandatory School Assessment, but it may be waived by taking the
Mandated STAR assessment.”)

Agreement (How
to create a student agreement in FRED)

3. At the
Meeting:
-
Assuming that the parent and the ES have already discussed the phone interview
questions and it has been determined that CW is a good educational option
for their student, proceed to enroll the student.
At
the meeting, go through each item on the sibling enrollment checklist, and
note any explanations below, retain the checklist in the student’s file in
case it is needed for ES or School Office future reference.
*Complete
the Student Agreement
*Write
sibling’s name on current family rollsheet and
show how to start marking first day of attendance. Draw lines through any
dates in the X's and O's portion of the roll sheet that do not pertain to
that student, and initial them. The beginning and ending dates written at
the top of the roll sheet do not need to be changed regarding this student,
just make sure that the correct number of days are written
for the "total days of attendance and total days of absence".
*Have
student complete Kaleidoscope Learning Styles profile (ESs may pick up copies
of the inventories at the learning centers) and a writing sample
(a short paragraph on why they want to attend our school, or something that
they want to say about themselves.).
