PORTFOLIO INFORMATION
A
portfolio is one work sample for each learning area covered that semester
if the student is in grades K-8 (if a K-8 student has learning recorded in
growth area on the learning record, a sample for that growth area must be
included) and one work sample per COURSE per semester for 9th to
12th. Students doing a mixture of K-8 and 9-12 may include all
samples in the same envelope, listing the samples correctly on the Portfolio
Checklist.
Samples
must be non-sectarian, must be from materials that are or could be purchased
by the school with
Samples
will not be returned, so we recommend copies of original artwork, videos,
and cassettes be submitted.
All
students enrolled must have a portfolio on file by the end of each year. If
a student has dropped and was enrolled in the school for 2 months or less,
and you are unable to obtain the full amount of samples required, then a minimum
of two samples (total) will be accepted.
1st semester portfolios will be reviewed
and checked for completeness at the December ES meeting. Portfolios
will be collected for both first and second semester at the April ES meeting.
Each portfolio
must contain samples from both semesters, if the student attends both semesters.
The ES should carefully choose samples that display a progress in
learning
from one semester to the next.
*
Samples must contain the following information in the upper right corner printed
on a portfolio label, Times New Roman 12 pt. font (1 X 2 5/8")
-Student

-ES

-Date
of collection

Using their ES supply funds, the ES will need to purchase a box of 10x13 manila
clasp envelopes for the portfolio's submission.
*
Each portfolio envelope must include the following information in the upper
right corner printed on a 2 X 4" label (Lay envelope horizontal, clasp
down, opening on right hand side).
-Student
Number -Student Name ( last, first )
-ES
Code -- -ES Name
-School

Year
(Labels are supplied for all ESs at the beginning of each school year. If more labels are needed, please contact your Area Facilitator. If an ES wishes to type and print their own portfolio labels, the information must be typed in Times New Roman 12 pt. font. in the same order as the above information. See ES typed portfolios for more information.)
When
the portfolio is ready to be assembled, place a FRED printed, completed PORTFOLIO
CHECKLIST ( in ink), one for each semester, with the appropriate
samples behind each checklist. Be sure to organize the samples in the same
order as the growth areas/courses appear on the checklist. (Extra samples
in a growth area can be placed at the end of the sample set).
Note: There is a button in FRED that states "print all" that may
be used to print all of your students' checklists. Once you click on it, it
may take 3-5 minutes to process before it starts printing. You should also
change the "print pages" on the print window from "1"
to "1-27" (or however many students you have).
Two
of the required graded writing
samples may be the Reading/Writing Growth Area samples and the other two
writing samples are kept by the ES in the students file. First drafts of writing
should be used if possible, and copies of the rubric are not required. The
samples should NOT be typed, but if desired, the typed final copy can be attached
to the students first draft.
Place
all the samples, including the checklists, into one appropriate student portfolio
envelope, and clasp the envelope closed (do not seal ). Please do not
staple items together unless they are part of a single sample and do not put
envelopes or folders within the portfolio envelope.
1st
semester portfolios will be due for review for all students at the December
ES meeting. Portfolios will be collected at the April ES meeting.
Portfolios
for January Graduates are due earlier, and will be turned in to the ES Advisor
with the Grad Packet.
