Household
Information Collection
Each family needs to complete and return the Household Information Collection form. The form must be updated annually for all families, even those who were newly enrolled at the beginning of the school year. This form can be done in one of two ways.
1) Prefilled Form in FRED (view/parent accounts)
In order to use the
prefilled form, FRED must have the correct household size filled in.
Household size should include any adults living within the home, as well as any
biological or adopted children, and any children being cared for through
kinship care. The household size should not include foster children. If
the number is incorrect, please change it by clicking on "# in
family". After checking that FRED has the correct household size, print
the form to give to your parent for completion at the next LR meeting, (see
below). If the household information is still required for the family,
there will be a link that says “HH Info” after the parents’ name that will take
you to the
2) Blank Form in FRED
Always have blank forms available at learning record
meetings in case there has been a change in household size that you were not
aware of or in case you find that the household size listed in FRED on
your preprinted form is not accurate. The blank form can be
printed from the view/parent accounts layout, by using the button at the top
that says “Blank HH Info Form”. If you must use this form, be sure that
the parent chooses the
At the Learning Record Meeting
The parent should be asked to complete the form and return it to you at the learning record meeting. Many parents who homeschool their children feel uncomfortable acknowledging their education level if it is not college or above. They may also feel uncomfortable about sharing their income level. Please let your parents know that this information is used anonymously by the state and is only used for school wide statistical purposes. No one will look at this data to judge whether a parent has the proper educational or income level to home school their child. In fact, the more highly educated and affluent our parents appear could possibly have a negative impact on how our school’s overall ranking is compared to other schools ranking. If our parents have equal education and income levels as parents in an affluent upper class area, the state will compare our school’s test scores with schools in those similar areas. Encourage the parents to feel free to fill out the survey honestly. Since the information is confidential please give the parent the form and a plain white envelope. Have them return the form to you on that day. ESes send the completed forms to IEM, 4535 Missouri Flat Road, Ste. 1A, Placerville, CA. 95667. These are best sent all at once in one large envelope and must be postmarked by November 2. If the parents leave the envelope unsealed, the ES may remove the form from the envelope before placing in the large envelope for mailing, but if sealed, leave it in the sealed envelope when placing it in the large envelope. If a parent tells you that they do not want to answer a portion of the form, ask them to fill it out as best as they are able and then place it in the envelope.
