General Information Sheet
The General Information Sheet is available filled out with all
of the student/
The General Information Sheet is to be given to each family
at the first meeting of each school year. It provides the parent
with most of the personal information they may need to use during the year: phone
numbers, their student and parent numbers, the important test dates for the
year, e-mail addresses, and basic information about the school.
