BYU Independent Study Program

BYU has an independent study program that offers non-sectarian courses to our students. Please note the following:

(from the vendor notes section in FRED) This vendor charges a non-refundable  $10.00 materials fee.  Please submit a separate PO to cover this expense as well as the class PO. ES include student name, ES email, Customer# 079743441, & SAT/ACT CEEB Code 053706, in body of this PO! Also ES must Go to http://ce.byu.edu/is/site/courses/purchase/Enroll.pdf to fill out online PO make sure the shipping address is the ESes not the family’s.

On the Enrollment form:
Customer Name: Connecting Waters Charter School
SSN: of the student
Phone and email: either student or ES, your choice whatever the family is more comfortable with (it is under the student information section of the form but BYU says either way, they just need a contact)

For Finals, if you are not yet a proctor for BYU you will have to register through BYU.  
Proctoring Information:

On Step two it says:
Step 2 of 6
Proctors may NOT be any of the following:

• University professors
• Elementary School educators and counselors
• Relative or friend of student
• Employer of student
• Personal tutor of student
• Ecclesiastical leader *
• Athletic coach, assistant coach, athletic administrator, or athletic academic counselor
• Someone with an invested interest in the student's eligibility to compete in extracurricular activities (i.e. sports, theater, choir, etc.)
  I certify that none of the above applies to me.

When you get to Step 2, you click the box at the bottom “I certify that none of the above applies to me” , if you have a multiple subject credential, BYU considers you to be the high school student’s advisor, NOT an elementary school teacher or counselor.
