Highly Qualified Teacher Learning Record and Report Card Documentation

 

 

Learning Record and Report Card Documentation: The purpose for the HQT notation in FRED is to provide evidence to the CDE (California Department of Education) and the District in the event of an audit that we are following the NCLB Federal Mandate and have provided each student access to a HQT. 

 

A. When creating a High School Report Card, to the right of Work Level, there is a box called “NCLB Teacher of Record”.

-A selection must be made here before any subsequent drop downs will appear.
-You must choose from one of the following for all core academic subjects listed below, and it will then show in the learning record documentation beside the course title.

-If it is a core course, and no HQT selection is made, FRED will default to your name. This must be changed unless you are qualified for HQT in the subject.

-For Non-Core courses like PE, Educ & Career, Driver's Ed, etc, choose "0" for n/a and leave the default with your; changing it to another ESes name or another choice like HQT vendor for a non-core class will result in an error on the report.

-For non-core community college courses, select “0” for n/a, leave the default with your name, and then choose Community college under external course to indicate the college that the student is attending.

1) HQT ES is Certified:only if you are certified as the HQT for this course; you must be ESEA approved for this subject.
2) HQT Online Course: only for approved online courses which a PO is made and the vendor is selected from these lists - HQT online vendors and a-g online vendors NOT to be used for Moodle/Elluminate Courses nor any courses taught by CWCS employees)
3.1) HQT CP Course (for any HQT CP set up in FRED that is a core course (not for non-core): HQT Moodle/Elluminate, HQT CP, HQT Learning Center, HQT Tutoring, taught by a CWCS employee)
3.2)HQT Portfolio Reviews:  only for HQT Individualized Portfolio reviews and HQT1 taught by CWCS employees (See C below)
4) Community College. be sure to fill out all three boxes for this. See info in External Courses
5) HQT Tutor Vendor (see http://www.ieminc.org/handbook/NCLB/HQTCAHSEE.htm only for those marked vendor on this list list and for which a PO is made, NOT for CWCS Employees)

B. The next box to select is directly under "Course Teacher". A drop down will show Community College, Distance Learning, HQT Vendor, and then a list of all CW Employees that are HQT certified. You must choose one of these options for numbers 1-6 above. Here is an example of how things should look using a student of Rebecca Rasmussen. (The Grade “0” is invalid as the student used for this example was a Kindergarten student):

The HQT notations should be included in all core growth areas on each learning record
-English/Language Arts, Reading- including reading interventions and CAHSEE English classes
-Mathematics- including reading interventions and CAHSEE Math classes
-Science: Biological Sciences, Chemistry, Geosciences, Physics
-Social Sciences: US and World History, Government, Economics, Geography
-Foreign languages(specific-French, English, German, etc)
-Drama/Theater (English credential)
-Visual Arts
-Music
-Dance (Physical Education credential)

C. To sign up for Individualized Access/Portfolio Review (for outside vendors, use service PO)

1. Go to the High School Report Card entry screen.
2. Select 3.2 from the ESEA dropdown (see ReportCard1-selectESEA)
3. Select HQT Teacher (see ReportCard2-selectHQTteacher)
4. A new screen will appear. Select the HQT Type (see ReportCard-selectHQTtype).
    HQT Type: 
            a.   Portfolio HQT1 is only for general courses for students who are proficient/advanced in STAR.
            b.   For a-g: If a student is attempting 5 credits, five sessions must be selected.
            c.   For general courses, non-HQT1: If a student is attempting 5 credits, two sessions must be selected unless more sessions are needed as discussed with the HQT.
5. A description of the HQT type will appear below. Read and confirm (see ReportCard4-confirm-description)
6. Click "Go Back" (see ReportCard5-go-back)

D.  Elementary students taking Algebra 1 (or any higher math) must have a HQT, even if the student is not receiving high school credit for the course. In FRED, at the bottom of the Grade School Report Card, is a field to enter the course information, including the ESEA teacher of record. Please follow the above procedures to select the appropriate HQT.

 

E. The below high school courses have been added to the Special Interests Growth Area. They should be used strictly for elective courses that do not require an HQT, and are not to be used for core courses required for graduation. If you have a student taking an elective course and they are using an HQT, do not use the below titles.
