ES Procedure

*Check to see if there is any old information left over from last year that should be deleted (email Leita lbarlow@connectingwaters.org immediately with this info).

*Check your students grade level, phone number, and address (if incorrect, email the appropriate dept to update with accurate information and cc Leita immediately).

*Select yes from the drop down list by “Will student attend Star testing?” unless there is an issue in which you would select “I have a concern”.

*Check your student’s assigned test site. If there is a valid need to change the test site, select “I have a concern” and write in the text box the change needed.

*If there are any concerns at all, select “I have a concern” and then select the specific concern, ie: transportation,  will not test in church building, etc. If your concern needs further explanation, write it in the text box.

*Enter in the emergency contact number.

*Enter whether parent will assist during testing.

*Enter whether student may be a behavior problem.

*Grades 7-11 must make selections from the EOC drop down lists. If your student does not meet the criteria needed to take an EOC, select “no EOC needed”.

*Grade 11: If student wants to participate in the EAP, select/enter “2008” from the drop down list (leave blank if they are not participating).

*Click on the PFT (PE test) tab and enter pertinent information regarding medical limitations.
